LJ Architectural Issue Help Needed!

Earlier this summer, LJ mailed questionnaires for its annual Architectural Issue to libraries with building projects completed between July 1, 2006 and June 30, 2007, which included links to an online form. Unfortunately, the online application crashed, and most of the project details were lost (ain't technology grand!). If you submitted an online form, please resubmit it at the original sites:
